In the first section of article which is addressed mainly to Polish-only speaking reader the pathophysiology of ascites in cirrhotic patients is reviewed. Special focus was made on the forward theory of ascites formation. Subsequently, the rationale of the therapeutic approach in patients with different stages of ascites, hepatorenal syndrome and spontaneous bacterial peritonitis is provided. Those recommendations are based on published recently AASLD and EASL practice guidelines, report of Consensus Conference of the International Ascites Club, the statement of the North American Study for the Treatment of Refractory Ascites and on recommendations of the most experienced group of researchers from Barcelona, Spain.
