Summary: The persistence of hepatitis B virus genome in HBsAg-negative individuals, termed occult HBV infection, has been a matter of discussion for years, but now it is supported by many publications. Occult HBV infection may be an important factor in different clinical situations, including transmission of the infection, reactivation of hepatitis B during immunosuppression and development of hepatocellular carcinoma. Several aspects of this occult infection, like its role in fibrosis progression, chronic inflammatory reaction or evolution to „cryptogenic” cirrhosis remain unclear, but the existing reports enhance the importance of this medical problem, which needs much more research.
