Imbalance between generation of free radicals and defense mechanisms leads to oxidative stress. In children after liver transplantation it results from ischemia-reperfusion, immunosuppression and post-transplant complications. Moreover, in the next years so as to increase the donor pool, we have to prepare for more challenging grafts vulnerable to oxidative injury, primary graft dysfunction and rejection. We discuss pathogenesis, complications and ways of action against oxidative stress in pediatric liver transplant recipients.
